TCAPS Montessori WALKING SCHOOL BUS PROGRAM
Safe Routes to School – Parental Consent Form for Participation in the Walking School Bus Program
Purpose and Explanation of the Program
The Walking School Bus program is designed to get communities to work together to get children to school actively and safely. Children who participate in the program walk to and from school in groups led by adult volunteers.
TCAPS Montessori students will be walking to and from TCAPS Montessori (1009 Oak St.) beginning on the International “Walk to School Day” on Wednesday, October 5th. After October 5th, our Walking School Bus Program will take place on every Wednesday and Friday both before and after school. Our Walking School Bus route is from the Reedemer Church (located on the corner of Wadsworth and 9th Street) to the school in the morning and from the school back to the Redeemer Church in the afternoon. Children can be dropped off at the church parking lot in the morning no earlier than 7:40 a.m. Students participating in the Walking School Bus (WSB) program will leave the church parking lot by 7:50 a.m. to begin walking to TCAPS Montessori. Students living within the neighborhood between the school and the church will be picked up as the “School Bus” walks past their home and will be dropped off at their home after school on the way back through. In the afternoon, students participating in the WSB program will leave the school just after dismissal and will arrive back at the church parking lot by 3:30 p.m., so please pick your child up at 3:30 p.m. If picking your child up from the church parking lot, you will need to sign your child out with one of our WSB adult volunteers.
Program Objectives The goals of the Safe Routes to School program include the following:
• Increase daily physical activity for children and adults in the neighborhood.
• Increase safety for pedestrians in the neighborhood.
• Reduce traffic in and around the neighborhood and school.
Potential Risks
The Walking School Bus Program is intended to reduce the risk of injury to children as they travel to and from school through the use of volunteer adult supervision.
Voluntary Consent
I certify that I have read this consent form or it has been read to me and that I understand the program and its risks. By signing this consent form, I agree to allow my child to participate in this program.

If I cannot be reached, I understand and agree that my son/daughter may be taken for medical assistance, and I agree that I will be solely responsible for any and all costs incurred as a result. I further agree to hold harmless TCAPS Montessori, its employees and any volunteer for this program for any injury that occurs to my child which is not the result of action or inaction by the district or its representatives.
